Terms of Service | Removable Restorations Dental Lab

1. Acceptance of Terms

By submitting a case, placing an order, or otherwise using the services of Removable Restorations Dental Lab ("the Lab", "we", "us", or "our"), you ("the Client", "Dentist", or "Practice") agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

2. Services

Removable Restorations Dental Lab provides dental laboratory services including, but not limited to, complete dentures, partial dentures, implant prosthetics, orthodontic appliances, repairs, and relines. All services are provided exclusively to licensed dental professionals. We reserve the right to refuse service at our discretion.

3. Case Submissions & Prescriptions

All cases must be accompanied by a completed lab prescription signed by a licensed dentist. The prescribing dentist is solely responsible for the accuracy of the prescription, including tooth selection, shade, occlusal scheme, and any special instructions. The Lab will fabricate restorations according to the prescription provided and is not responsible for outcomes resulting from inaccurate or incomplete prescriptions.

4. Turnaround Times

Standard turnaround times are estimates and not guarantees. Rush service is available upon request and may be subject to additional fees. The Lab is not liable for delays caused by incomplete case submissions, shipping carrier delays, or circumstances beyond our control.

5. Remakes & Warranties

We stand behind the quality of our work. If a restoration does not meet the specifications of the original prescription, we will remake it at no charge within 30 days of delivery. Remakes requested due to changes in the prescription, patient preference, or clinical factors outside the Lab's control may be subject to additional fees. No warranty is provided for repairs to restorations not originally fabricated by Removable Restorations Dental Lab.

6. Payment Terms

Invoices are due upon receipt unless a credit account has been established. Accounts past due by more than 30 days may be subject to a late fee and suspension of services. The Lab reserves the right to require prepayment for new accounts or accounts with a history of late payment.

7. Limitation of Liability

The Lab's liability for any claim arising out of or related to our services shall not exceed the amount paid for the specific case giving rise to the claim. We are not liable for indirect, incidental, or consequential damages, including lost profits or patient injury resulting from the use of our restorations.

8. HIPAA Compliance

Removable Restorations Dental Lab operates as a Business Associate under HIPAA. We handle all patient information in accordance with applicable federal and state privacy laws. Patient identifiers should be limited to what is necessary for case fabrication. We do not share patient information with third parties except as required by law or necessary to complete the services requested.

9. Governing Law

These Terms of Service shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions. Any disputes arising under these terms shall be resolved in the courts of Dallas County, Texas.

10. Changes to Terms

We reserve the right to update these Terms of Service at any time. Continued use of our services following any changes constitutes acceptance of the revised terms. We encourage clients to review this page periodically.
